Benefits depend on the state and transaction
Medical certification or a state credential can provide access to a medical program. Some states treat eligible medical purchases differently for tax purposes, or permit a qualified patient to use a medical home-growing route. The four examples below describe current state rules checked 2026-10-23; they are not a promise of the same benefit nationwide.
Ohio
Medical access through licensed dispensaries, possible dispensary discounts, and exemption from the 10% non-medical excise tax. Official state source
Limit: Employer drug-free workplace policies may still apply.
Oregon
Eligible OMMP cardholders may make qualifying medical purchases at OLCC retailers without recreational marijuana tax. Official state source
Limit: Tax treatment depends on a qualifying purchase and valid program status.
Missouri
Medical purchases carry a 4% cannabis tax rather than the 6% adult-use cannabis tax. Official state source
Limit: Other applicable state and local sales taxes may still apply.
New York
A clinician-issued certification gives access to registered medical dispensaries; qualified patients and caregivers can use the medical home-grow route under state rules. Official state source
Limit: No physical state card is issued; certification and photo ID are required at checkout.
In Missouri, the 4% medical and 6% adult-use figures are cannabis taxes; other sales taxes may still apply. In Oregon, tax treatment depends on a qualifying medical purchase. New York uses certification rather than a physical state card.
Will it save money?
Possibly, but a lower purchase tax or a dispensary discount is only part of the calculation. Compare the clinician evaluation and recertification charges, any state or processing fee, product prices, purchase frequency and actual taxes at checkout. See the eight-state program fee comparison. No savings amount or universal break-even point is claimed.
What medical status does not guarantee
It does not guarantee workplace protection. Ohio’s regulator says employers retain discretion over drug-free workplace policies. Ohio DCC
Medical status is also not permission to carry cannabis across state lines. Check visitor rules and home-growing rules separately.
