What still needs checking
- Act chapter 1166 removed residency effective July 1, 2026; the older HHS patient page still uses resident-only wording. A nonresident still needs an Iowa card to buy; out-of-state card possession recognition is not dispensary reciprocity.
- Specific medical checkout tax treatment remains unresolved; no rate is asserted.
Possession and use
Iowa's medical program protects registered patients and caregivers using products permitted under Iowa Code chapter 124E. It is not a general marijuana possession exemption. For products dispensed through the program, the default limit is 4.5 grams of total THC over 90 days unless an authorized health-care practitioner provides a waiver; certain terminal conditions may also qualify for an exception. This is a THC-content limit, not 4.5 grams of flower. Ordinary adult-use marijuana remains outside the program.
Medical access
A patient with a qualifying condition must obtain certification from an authorized practitioner and apply to Iowa Health and Human Services for a medical cannabidiol registration card. Iowa's 2026 Act chapter 1166 removed the patient residency requirement effective July 1, 2026, so an otherwise qualified nonresident may apply. The card is generally valid for one year, with recertification needed for renewal. Separate limited recognition of an out-of-state card for compliant possession does not itself grant Iowa dispensary purchasing access.
Home growing
The Iowa patient card does not allow home cultivation of marijuana. Licensed manufacturers produce regulated medical products for licensed dispensaries; the patient and caregiver roles do not include growing plants. Iowa also has no adult-use home-grow allowance. Industrial hemp and consumable hemp are regulated separately, with their own registration and product rules; those systems should not be mistaken for permission to cultivate marijuana for personal medical use.
Where to buy
Iowa HHS lists five licensed medical cannabis dispensaries. A registered Iowa patient or caregiver buys permitted products under the program and the 90-day THC allotment. Products can include formulations containing both CBD and THC, but ordinary smokable marijuana flower is not made available through this program. A retail hemp seller operates under different rules and is not one of the state's licensed medical dispensaries. Check the HHS directory and your card's status before traveling.
Taxes and price
The official patient and dispensary pages establish access and quantity limits, but they do not provide a clear statewide cannabis-specific excise amount or a final tax-inclusive price for medical purchases. This guide therefore does not invent a medical tax percentage. General Iowa sales tax, any applicable exemption and the product's regulated status require a current Department of Revenue or dispensary receipt check. Iowa has no adult-use marijuana market from which to quote a recreational excise rate.
Travel and public places
Do not carry cannabis across a state border on the theory that a medical card from either state automatically solves the issue. Iowa's limited out-of-state recognition in law-enforcement guidance concerns Iowa-compliant possession, not out-of-state dispensary access or federal interstate transport. Use only products allowed by Iowa's medical rules, carry registration documentation and avoid impaired driving. An adult-use purchase made in another state does not become an Iowa medical product after crossing the border.
